New Patients

If you live in the Hafan Iechyd Surgery catchment area (Caernarfon, Bontnewydd, Llanwnda, Rhostryfan, Rhosgdfan, Groeslon, Carmel, Pontrug, Bethel) you can apply to register at Hafan Iechyd.  New patient information packs are available to collect from our reception.  Alternatively, you can download the new patient registration form GMS1w from here GMS1w New patient registration

Please remember to sign the GMS1w form before returning it to Hafan Iechyd and enter your NHS number.  Your NHS number can be obtained from your current GP. 

Proof of identity is required.  One item from each column in the document below must be produced at the reception for verification purposes only.

Under 16’s

Children under the age of 16 whose Parent/ Guardian is registered with the practice/ registering at the same time will need to provide either:

• Original Birth Certificate or a certified copy
• Passport

If you are unable to provide any of the above documents please speak to a member of the reception team who will be able to discuss alternative documents.
